• More Flexibility: You can choose who provides your care—including a spouse or parent of a minor.
• Two Models of Care:
- Agency Model: We manage everything for you—staff hiring, training, payroll, and scheduling.
- Budget Model: You take the lead as the employer of your workers, with support from a Financial Management Services (FMS) provider.
• Goods & Services: Use your budget to purchase approved items like emergency response systems or assistive technology.
• Training Budget: Participants receive funds for support worker training and development.
• Consultation Services Provider: We help build a personalized service plan to fit your goals and needs.
You may qualify for CFSS if you:
• Are enrolled in Medical Assistance (MA), a waiver program, Alternative Care, or MinnesotaCare
• Live in a home or community setting (not a facility)
• Need assistance with daily living
• Can direct your care—or have a representative to do so
• Complete a MnCHOICES assessment
As of October 1, 2024, CFSS has replaced the PCA and CSG programs. If you’re already receiving PCA services, your care will continue without interruption until your next assessment, when you'll be transitioned to CFSS.
Our respite care services offer temporary relief for family caregivers, allowing them to take a break and recharge.
Our transitional care program helps patients transition from hospital to home, with a focus on medication management and follow-up care.
